Inconsistent/too quiet volume over radio and proximity VOIP

Issue/bug you are reporting: 

VOIP has been beyond inconsistent over the past 9 months at least. I can't exactly pinpoint when it became an issue, but from talking with others, it seems to have coincided with the update to whisper range.

  1. When logging in, it's a dice roll who will be randomly barely audible.
  2. The issue is present both in proximity and radio VOIP, however, it's more pronounced over radio.
  3. Others changing their mic volume does not always make a difference.
  4. Restarting the game sometimes fixes it but not always.

Expected behavior: Being able to consistently hear people at a reasonable volume.

I have not found any steps or conditions to consistently replicate it and without knowing the exact implementation of VOIP on the server, I'm only going to be guessing as to the root cause:

  1. Distance vectors not updating on log in / area stream and/or in conjunction with user selected voice range.
  2. Distance vectors for whisper being applied in some quirky way as a dot product when you log in / stream an area to radio VOIP.
  3. An issue with volume calculation itself such as values expecting the old whisper range.

Yes, this has been happening a ton. As well as sometimes one person can hear someone talking — either nearby or over the radio — but someone else close by or on the same radio frequency can't hear them at all. It usually fixes itself when the person stops and starts speaking again. I personally have only noticed this issue within the past 2-3 months or so, I don't recall this happening earlier than that.
